How to File Your Utah Annual Report
To successfully file your Utah Annual Report, follow these steps:

- Complete your report online OR print a paper form.

Late Fees: Utah will assess most businesses a $10 penalty fee for failing to file an annual report. The exception is LLLPs. They won’t be charged a late fee. All delinquent business entities will risk being administratively dissolved or revoked. |

Filing Your Utah Annual Report
The Utah Annual Report can be filed online or by mail. No matter how you choose to submit your report, you’ll need to visit the Utah Division of Corporations and Commercial Code website.
- On the state website, hover your mouse over the “Business Entities” tab.
- Select your appropriate business type from the drop down menu.
- In the box labeled “Online Services” click the “Renew” link.
To File Online:
- Click the “Renew Your Business Online” button.
- You will be prompted to enter your Entity Number and Renewal/Access ID. You can find your entity number by searching the Utah Business Database. To request a renewal/access ID, click “Receive an Access ID” OR call the Division of Corporations at (801) 530-4849.
To File By Mail:
- Click the “Renewal Application” link. All business entities use the same annual report.
The following is a complete list of everything you’ll need to include/verify on your Utah Annual Report:
- Business entity number.
- Business type.
- Date your renewal is due.
- Business name.
That’s it! Utah Annual Reports are for renewing your business name. If you need to make any updates or changes to your business information, you’ll need to file a Registration Information Change Form. There is no filing fee when it’s submitted with the annual report. Otherwise, it costs $15.
Online submissions must be paid with a credit card. Mailed reports must include a check or money order payable to “State of Utah.”
