Filing Your Wisconsin Annual Report
Most Wisconsin Annual Reports can be filed online OR by mail. The exception is LLPs, who can only file by paper. To obtain a paper form, you must email the Department of Financial Institutions at [email protected]. All other entities must visit the Wisconsin One Stop Business Portal OR Annual Report ID Lookup page – depending on what type of business you own.
To File For Domestic Corporations and LLCs:
- On the Wisconsin One Stop Business Portal, click “File My Annual Report Today.”
- Enter your business name and click “search.”
- Select your business from the search results. To continue with online filing, click “Next.” To file by mail, click the paper form link.
To File For Nonprofits, Foreign LLCs, and Foreign Corporations:
- On the Wisconsin Annual Report ID Lookup page, enter your business name in the search box.
- Then, select your business from the search results.
- Click “start.” You’ll immediately be re-directed to the Wisconsin One Stop Business Portal – If you try starting out at the portal, it won’t let you file. We don’t understand why either.
- To continue with online filing, click “Next.” To file by mail, click the paper form link.
Here is a list of information you will need to include, verify, and/or update on your Wisconsin Annual Report:
- Business name.
- Registered agent name and address.
- Officer/director, manager, or partner names, titles, and addresses. LLCs are not required to include member information.
- Corporation stock information. Include class, series, number of shares authorized, and issued/outstanding.
- Business email address.
- Signature of person filing the report.
Online filings must be paid with a credit card. In addition, you’ll pay a $1 processing fee. Mailed filings must include a check or money order.
