Your Washington DC LLC may be administratively dissolved if it fails to do any of the following:

• pay overdue penalties or fees for five months
• submit biennial reports for five months

• your entity name
• an alternative name, if your old name has been adopted by another company
• the date your LLC was administratively dissolved
• the date of filing

The form also includes a statement that your LLC has appointed a registered agent and is current on biennial reports and fees. It must be signed by a manager, member, or authorized person.
